Co-op Status Forms — Students

Please submit all completed forms by e-mail to Co-operative Education at coopadmin@uoguelph.ca

Co-operative Education In-course Admission Request for First-Year Students:

  • Co-op in-course applications are submitted through the Experience Guelph portal.

  • For details on eligibility, timelines, and how to apply, please visit our Co-op Admissions page.

Co-operative Education External Transfer Student Admission Request:

  • Co-op External Student Admission Request Package
    Students studying at another post-secondary institution cannot apply directly for admittance to co-op at the University of Guelph. After being accepted to the university and determining the number of transfer credits, eligible students may submit this application. Further information can be found here.

Co-operative Education Transfers Request:

If you're requesting to change your major, please follow these steps:

  1. Complete the Academic and Work Sequence Agreement.
    • Send it to your Academic Advisor for review and approval.
    • Email the signed form to coopadmin@uoguelph.ca.
  2. Submit the Schedule of Studies Change Request form. Your Schedule of Studies Change Request form submission will be reviewed by the Co-operative Education Team following approval by your Academic Advisor.

Co-operative Education Reinstatement Request:

  • Co-op Reinstatement Request Package
    Students who have automatically been withdrawn from their program's co-op option when changing degree programs or when deferring admission to a Winter term due to a study permit delay, may request reinstatement into co-op by submitting this package within the semester in which the removal from co-op occurred.

Co-operative Education Academic & Work Sequence Agreement:

  • Co-op Education Academic & Work Sequence Agreement Form
    Students must complete this form if applying to co-op, changing specialization/major, or changing degree, or if needing to request to deviate from the prescribed sequence.

Undergraduate Schedule of Studies Change Form:

  • Undergraduate Schedule of Studies Change Form
    If changing majors, you must submit this form with the Admission or Transfer Request package.

Co-operative Education Withdrawal Request:

  • Co-op Withdrawal Request Form

Request for Co-operative Education Consideration Appeals Request:

  • Request for Co-op Consideration Appeals Form
    Students wishing to appeal being withdrawn from Co-op due to a reason that falls under Work Preparation and Work Term Appeals must submit this form within 15 class days following the date they received the e-mail notification of the withdrawal.


Co-op Work Term Forms — Students and Employers

Co-op Work Term Report Employer Comments:

  • Co-op Work Term Report Employer Comments Form
    Submit this form, along with your work report, by the 5th class day with your supervisor's signature.

Co-op Work Performance Evaluation:

  • Co-op Work Performance Evaluation Form
    Employers are encouraged to complete evaluations online. However, if this is not possible, students can forward this form to their supervisor.

Co-op Job Confirmation and Re-appointment Process:

  • Co-operative Education Work Term Approval Request Form
    If you have confirmed a job outside of the Experience Guelph process, or you are returning to your current/previous employer, you must submit all applicable work term information.
